This is the checklist for required student-athlete documents. Additionally, there are links to most of the documents so that they can be printed out, completed, and returned to the IVCC Athletics Office. If a student-athlete has any questions, please contact the Athletic Department at 815-224-0472.
CHECKLIST:
1. IVCC Student-Athlete Insurance Requirements and Forms: These forms explain the minimum requirements for student-athlete insurance in order to participate in IVCC Athletics, as well as the forms that must be completed by the student-athlete and parent(s) and on file within the Athletic Department. Please read these forms thoroughly- Medicaid and the State of Illinois (or any other state sponsored programs) - Healthcare and Family Services Medical Card do not qualify as primary insurance.

2. IVCC Student-Athlete Emergency Consent Form: This form provides emergency information and consent for care in the case of emergency. It must be completed by the student-athlete and parent(s) and on file within the Athletic Department

3. IVCC Student-Athlete Physical Form: IVCC will provide student-athletes with physicals at the IVCC Gymnasium. However, if a student-athlete would like to obtain their own physical from their own physician, they will be responsible for paying for the physical. The attached document can be printed and given to the medical provider. The completed form must be on file within the Athletic Department before the student-athlete can participate in any practices or competitions.
4. NJCAA Student-Athlete Affidavit Form: This document is intended to collect important information about each student-athlete in order to assist colleges in determining athletic eligibility. This must be completed and on file within the Athletic Department.

5. FERPA Consent To Release Educational Information: This document is needed to provide consent for IVCC to share educational information with the NJCAA.

6. High School Transcripts: Official High School Transcripts with a graduation date posted must be sent to the IVCC Records Office from the student-athlete's high school.
7. College Transcripts: Official College Transcripts from EVERY COLLEGE ATTENDED (this includes dual credit courses, full-time, part-time, online, etc.) must be sent to the IVCC Records office from the college where classes were attempted. Even if a student did not pass or complete classes, a transcript must be sent.
8. Impact Concussion Test: IVCC will provide required baseline concussion testing to all incoming student-athletes. These tests are valid for 2 years. Sessions for these will be posted prior to the start of school each year.
